Integrative psychiatry · San Diego, CA

Care that treats the whole system, not just the diagnosis.

Evidence-based psychiatric mental health care grounded in integrative and functional medicine, ketamine-assisted treatment, and narrative-focused depth work, available in person and via telehealth for patients across San Diego and the surrounding coastal communities.

You might recognize yourself in one of these

You're succeeding by every outside measure, and quietly falling apart.

You've gotten so good at holding it together that people stopped asking if you're okay. You want somewhere that sees past the performance.

You've cycled through medications and something is still missing.

You want a prescriber who looks at your labs, your sleep, and your history, not just the last thing that didn't work.

You're managing a diagnosis, but you barely recognize your own story anymore.

You want room to make sense of what happened to you, not just a plan to manage what it left behind.

You're curious about ketamine-assisted treatment, but don't know where a real conversation starts.

You want a grounded, honest clinical conversation, not hype, and not a closed door.

Three threads, one practice

Medication management is one tool among several. Each patient's plan draws on the threads below in whatever mix actually fits their history and goals.

01

Integrative & functional psychiatry

Medication decisions considered alongside sleep, nutrition, metabolic, and hormonal health: the whole physiology, not the symptom in isolation.

02

Ketamine-assisted care

Thoughtful, medically supervised use of ketamine as part of treatment for select patients, delivered within scope and current regulation.

03

Narrative & depth work

Room to make sense of your own story alongside clinical treatment, because symptom relief and meaning-making aren't separate projects.

What guides the work

Curiosity

Symptoms are information, not just targets. I stay curious about what's actually driving them before reaching for a prescription pad.

Whole-person care

Your physiology, your history, and your relationships are connected. Treatment that ignores one usually falls short on the others.

Transparency

You should always understand why a medication, a test, or a referral is being suggested, and be free to ask why not.

Collaboration

You're the expert on your own life. Treatment plans work best when they're built with you, not handed to you.

Care that doesn't separate mind from body

Mental health care has traditionally been split into separate boxes: therapy or medication, mind or body, emotional or physical. But people don't actually work that way. Sleep affects mood. Stress affects digestion. Hormones shift emotional regulation. Trauma lives in the nervous system as much as the mind. Integrative care starts from a simpler premise: you're a whole person, and treatment should reflect that.

Biological

Brain chemistry, hormones, inflammation, genetics.

Psychological

Thought patterns, coping style, attachment, trauma history.

Social

Relationships, environment, day-to-day stressors.

Meaning and identity

Values, purpose, and the parts of life that make it feel worth living.

Medication has a real place in this approach, but it's one tool among several, not the default starting point. When it makes sense, it's chosen deliberately and reevaluated over time, guided by your labs, history, and how you're actually responding rather than prescribed on autopilot. Some people need medication support. Others do better with therapy, nutrition, or nervous system work. The goal isn't to use every tool available. It's evidence-based care built around what actually fits you, decided with you rather than for you.

Lucas Jacobsen is a board-certified Psychiatric Mental Health Nurse Practitioner who believes that symptoms are signals, not the whole story. Real healing begins when we look beyond the surface and address the deeper causes of dysfunction. He earned his MSN from Vanderbilt University and completed a post-graduate residency through the VA, where he worked with complex psychiatric presentations across a broad range of ages and life experiences.

Lucas has a particular passion for working with people who have learned to perform rather than feel, those who are high-functioning on the outside and quietly struggling on the inside. He brings warmth, directness, and genuine curiosity to each session, holding space for both the science of the brain and the story of the person in front of him.

His approach blends integrative and functional psychiatry with trauma-informed, whole-person care, drawing on psychopharmacology, nutritional psychiatry, advanced lab testing, and pharmacogenomic insights to understand not just what you're experiencing, but why. Whether that means exploring gut health, micronutrient status, or your personal history, Lucas builds care plans that support lasting change rather than simply quieting symptoms.

Before becoming a clinician, Lucas thru-hiked the Pacific Crest Trail, 2,650 miles from Mexico to Canada. Over five months he walked alongside people from five different continents, each carrying their own beautiful and sorrowful stories of joy, grief, and suffering. He came to understand that the human experience is far more universal than it is unique, and that most people are simply doing their best with what they have. That understanding, and the humility it requires, is something he carries into every clinical encounter.

He cares for a wide range of patients, including those navigating mood disorders, anxiety, PTSD, OCD, ADHD, depression, substance use, sleep issues, gut-brain dysfunction, PMDD, and chronic stress or burnout.

He currently practices in collaboration with Luma Health and Wellness and Mission Psychiatric, under the standardized procedures of collaborating physician Dr. Troy Kurz, MD, while completing hours toward independent practice authority under BRN Section 103.
